I installed Office 2003 SP3 and the problem disappeared. The culprit in my case was a software called "SnagIt". Roughly about the same time as I posted the problem I also sent an email to Microsoft via their web site giving all the information including product ID etc.
Its found in doc & settings\user\application data\microsoft\office\recent Just delete the contents of the folder.

Do you run Outlook in Cached mode? ¬†I've seen instances where the OST and offline settings files¬†cause Outlook to crash. Your cache administrator is webmaster. Additionally, Word will have reset many settings that you have customized in the Options dialog box. After deleting the "normal.dot" file, the problem was solved.
